In short
- Each Mounjaro KwikPen holds 4 weekly doses, so a 3-pen bundle covers 12 weeks
- The 2.5mg starting dose is meant for 4 weeks only, so a multi-pen 2.5mg bundle rarely fits the dose schedule
- Each dose step needs at least 4 weeks before the next increase, and some people stay longer or step back
- Unopened pens stay in the fridge at 2C to 8C until their expiry date, and only one should be in use at a time
- After first use, a pen lasts up to 30 days out of the fridge at no more than 30C
A Mounjaro bundle is worth considering only once you are settled on a maintenance dose, tolerate it well, and plan to stay with the same pharmacy for the next 2 to 3 months. In that case the lower cost per pen and fewer deliveries can add up. In the first months, while your dose rises every 4 weeks, a bundle risks leaving you with pens you cannot use. Dispensed medicines generally cannot be returned, so any pen you do not use is money lost.
What a bundle is
A bundle is a single order for 2 or 3 Mounjaro KwikPens instead of one. Each pen holds 4 weekly doses, so:
| Bundle | Doses | Weeks covered |
|---|---|---|
| 1 pen | 4 | 4 |
| 2 pens | 8 | 8 |
| 3 pens | 12 | 12 |
Most bundles contain pens of one strength. Some pair strengths in sequence, such as a pen at your current dose followed by a pen at the next step up. The patient leaflet lists packs of 1 and 3 pens, though not every pack size is sold in every country. In practice a bundle may arrive as separate single-pen packs.
How bundles are priced
Pharmacies price bundles to reward a larger order. The usual pattern is a lower price per pen than a single pen bought each month, and one delivery charge instead of two or three. You pay for all the pens up front.
To compare fairly, work out two numbers for any offer:
- Cost per weekly dose: the total price, including delivery, divided by the number of doses.
- The same figure for single pens at the same pharmacy, bought every 4 weeks with delivery each time.
The gap between these is your real saving. Check whether the single pen price you compare against is a first-order price or the normal repeat price, since these often differ. The guide to Mounjaro prices explained covers the parts that make up a price, and you can compare UK Mounjaro prices across pharmacies.
The dose change problem
The SmPC sets out a fixed way to start. You take 2.5mg once a week for 4 weeks, then move to 5mg. After this, the dose can rise in 2.5mg steps to 7.5mg, 10mg, 12.5mg and 15mg, with at least 4 weeks on each dose before the next increase. The dose steps guide explains the reasons.
This schedule clashes with bundles in three ways.
The starting dose lasts one pen
The 2.5mg strength is a starting dose, not a maintenance dose. One pen covers the full 4 weeks. Three 2.5mg pens would keep you on the starting dose for 12 weeks, which is outside the normal schedule.
You cannot predict the next 12 weeks
Some people move up every 4 weeks. Others stay on a dose for longer because of side effects or because the dose is working. Some step back down. If you buy 3 pens at 7.5mg and your prescriber moves you to 10mg after one, two pens are at the wrong strength.
You cannot split or combine doses
Each pen delivers one fixed dose per click. The leaflet says not to change your dose unless your doctor tells you to, and not to try to use leftover medicine in a pen. You cannot reach 10mg by combining a 7.5mg and a 2.5mg dose from different pens without your prescriber's instruction.
Storage and expiry
More pens means more to store correctly. The patient leaflet says:
- store pens in a fridge at 2C to 8C
- do not freeze them, and do not use a pen that has been frozen
- after first use, a pen can be kept out of the fridge at no more than 30C for up to 30 days, then must be thrown away
- the expiry date after EXP on the label and carton refers to the last day of that month
Keep unopened pens refrigerated until you start them, and start only one at a time. Check the expiry date on every pen in a bundle when it arrives, and use the earliest-dated pen first. Make room at the back of a shelf, away from the freezer compartment and from any part of the fridge where food freezes.
If you travel during the bundle period, you will need to carry or store the spare pens cold. A fridge failure or a long power cut affects every pen you hold, not one.
Prescriber reviews
GPhC guidance expects online prescribers to keep checking your progress when they supply a weight loss medicine over time. For repeat orders, your weight and progress should be checked and verified periodically. A bundle stretches the gap between reviews.
Many prescribers will not supply a multi-pen bundle at the start of treatment or during dose increases for this reason. Expect a review question at each order about side effects, weight and any new medicines.
If you need to stop
Some events mean stopping Mounjaro straight away or pausing it:
- suspected pancreatitis, with severe stomach pain spreading to your back
- planning a pregnancy, since the MHRA advises stopping at least 1 month before trying to conceive
- an unexpected pregnancy
- an operation needing general anaesthesia or deep sedation, where your team may give you specific advice
- side effects you cannot manage
Any unopened pens from a bundle are then unused. Prescription medicines, once dispensed, generally cannot be returned for reuse, so most pharmacies will not refund them. Read the refund terms before ordering. Take unwanted pens back to a pharmacy for safe disposal rather than putting them in household waste.
When a bundle can make sense
A bundle is a reasonable choice if all of these apply:
- you have been on the same dose for at least 4 weeks and plan to stay on it
- side effects are mild or settled
- your prescriber agrees to a multi-pen supply
- you have fridge space and no long trips planned
- you intend to stay with the same pharmacy for the bundle period
Buying several bundles, or ordering from more than one provider at once, is a different matter. In August 2025 the Royal Pharmaceutical Society strongly advised against stockpiling Mounjaro because of pressure on supply, and each prescriber needs a full picture of what you take.
A bundle can also protect you against a short-term stock problem at a stable dose. The guide to supply shortages explains what to do if your strength becomes hard to find.
When a single pen each month is safer
Buy one pen at a time if:
- you are starting on 2.5mg
- you are still moving up through the doses
- you have ongoing nausea, vomiting or other side effects
- you are planning a pregnancy or have surgery coming up
- you are unsure whether you will continue treatment
- you may switch provider, since the guide to switching pharmacy explains why records and timing matter
Avoiding a gap between pens
Whether you buy one pen or three, order the next supply before your final dose. Each pen gives 4 doses, and the leaflet tells you not to try to use what is left in the pen afterwards. The guide to the fifth dose question shows how to time repeat orders so you do not miss a week.
Questions people ask
Is it cheaper to buy Mounjaro in bundles?
Often the price per pen is lower, and you may pay for one delivery instead of several. Work out the cost per weekly dose by dividing the bundle price by the number of doses, then compare it with single pens bought monthly, including delivery each time.
Should I buy a Mounjaro bundle when I start?
Usually not. The 2.5mg pen is a starting dose used for 4 weeks before moving up to 5mg. Several 2.5mg pens would keep you on the starting dose too long, and you cannot yet know how you will tolerate 5mg.
Can I return unused Mounjaro pens from a bundle?
Generally no. Once a pharmacy dispenses a prescription medicine, it cannot be reissued to someone else, so most pharmacies will not refund it. Check the terms before you order. Unwanted pens should go back to a pharmacy for safe disposal.
How should I store the extra pens in a bundle?
Keep every unopened pen in the fridge at 2C to 8C, away from the freezer compartment, and check the expiry date after EXP on each one. Start one pen at a time. Once in use, a pen can stay out of the fridge at up to 30C for 30 days.
What happens to my bundle if my dose changes?
The remaining pens are at the old strength. You cannot safely combine or split doses to reach a new strength. If your prescriber moves you up or down, you will need a new pen, and the old ones may go to waste.
