Dry mouth and bad breath are not listed side effects of Mounjaro, but plenty of people get them. The usual causes are drinking too little, ketone breath from eating far less than usual, and reflux or burping, which are common side effects. Sip water across the day, eat regular small meals and keep up your dental care. Most cases clear within a week or two.
Why your mouth feels dry
The SmPC and patient leaflet do not include dry mouth. The NHS lists dehydration and medicines among the main causes, and dehydration is the one to check first on Mounjaro.
Tirzepatide reduces appetite, and many people find thirst drops at the same time. Less food also means less water, because food supplies a good share of your daily fluid. Add vomiting or diarrhoea during dose increases and fluid losses rise fast. The leaflet warns of dehydration from vomiting, nausea and diarrhoea and tells you to drink plenty of fluids.
Other causes the NHS lists:
- Breathing through your mouth at night, from a blocked nose or sleeping with your mouth open
- Anxiety
- Other medicines, such as some antidepressants, antihistamines and blood pressure tablets
- Oral thrush
- Diabetes, if blood sugar runs high
A dry mouth that comes with needing to pee often and feeling very thirsty can be a sign of high blood sugar. Ask your GP for a check, especially if you have not been tested for diabetes.
Three causes of bad breath on Mounjaro
Ketone breath
When you eat far less than usual, especially little carbohydrate, the body burns fat for fuel and produces ketones. Some are breathed out. The smell is often described as fruity, sweet or like nail polish remover. The NHS lists fasting and crash dieting as causes of bad breath.
Ketone breath is a signal you are eating too little. Regular meals with some starchy carbohydrate, such as a slice of wholemeal toast, a small baked potato or a portion of porridge, usually clear the smell within a few days. Read eating too little on Mounjaro for minimum intake guidance.
A dry, coated mouth
Saliva washes away food and bacteria. With less of it, bacteria build up on the tongue and between teeth and give off sulphur compounds. Morning breath is worse because saliva flow drops overnight.
Reflux and sulphur burps
Reflux, indigestion and burping are each listed as common, affecting up to 1 in 10 people. Tirzepatide slows stomach emptying, so food sits longer and gases form. Acid and gas rising into the throat leave an eggy or sour smell. The burps, bloating and reflux page covers this in detail.
Fixing a dry mouth
The NHS suggests:
- Sip cold water regularly through the day
- Keep a glass of water by the bed at night
- Suck ice cubes or sugar-free ice lollies
- Chew sugar-free gum or suck sugar-free sweets to get saliva flowing
- Use lip balm if your lips are dry and cracked
- Ask a pharmacist about saliva substitute gels, sprays or lozenges
And to cut down on:
- Caffeine, alcohol and fizzy drinks
- Acidic, spicy, salty or sugary foods
- Smoking
A practical fluid target is 6 to 8 glasses a day, with pale yellow urine as your guide. See staying hydrated on Mounjaro for ways to drink more when you never feel thirsty.
Fixing bad breath
The NHS steps for bad breath, adapted for life on Mounjaro:
- Brush teeth and gums for 2 minutes twice a day with fluoride toothpaste
- Clean your tongue once a day with a tongue scraper
- Clean between your teeth once a day with interdental brushes or floss
- Spit after brushing, but do not rinse with water straight away, so the fluoride stays on your teeth
- Use an antibacterial mouthwash at a different time from brushing
- Eat regular small meals so ketones do not build up
- Finish eating at least 3 hours before bed to reduce night reflux
Sugar-free gum helps both problems. Chewing gum does make you swallow air, so go easy if you are already bloated.
Protecting your teeth
Saliva neutralises acid and repairs early damage to tooth enamel. A dry mouth raises the risk of decay. Reflux and vomiting bring stomach acid into the mouth and wear enamel away.
After vomiting, rinse with water and hold off brushing for a while, because brushing soft, acid-covered enamel wears it faster. Keep up dental check-ups. Tell your dentist you take Mounjaro and have had a dry mouth or reflux.
If you sip drinks all day to stay hydrated, choose water, milk, or tea and coffee without sugar. Sipping fruit juice, squash with sugar or fizzy drinks for hours keeps teeth under constant acid attack.
When to get help
Call 999 or go to A&E if:
- You have diabetes and fruity-smelling breath comes with deep or fast breathing, vomiting, stomach pain, drowsiness or confusion. These are signs of diabetic ketoacidosis.
- You have a very dry mouth with confusion, cold or blotchy skin, or fast breathing (signs of severe dehydration)
Call 111 or see a GP today if:
- You cannot keep fluids down and your urine is dark or you are peeing much less than usual
- You feel dizzy on standing and the dizziness does not pass
- Your mouth is painful, red, swollen or bleeding, or has sore white patches
Mention at your next check if:
- Your mouth stays dry after a few weeks of self-care
- Bad breath has not cleared after a few weeks of good brushing (see a dentist)
- You have painful or bleeding gums, toothache or a loose tooth (see a dentist)
- You are thirsty all the time and peeing a lot, and want a diabetes check
- Reflux keeps coming back
The NHS advises seeing a dentist for bad breath lasting more than a few weeks despite self-care. You can report any side effect you link to Mounjaro through the MHRA Yellow Card scheme.
Questions people ask
Does Mounjaro cause dry mouth?
Dry mouth is not listed as a side effect in the UK leaflet. It is still commonly reported, usually because people drink less when appetite and thirst drop, or lose fluid through vomiting and diarrhoea. Sipping water through the day and keeping a glass by the bed at night usually fixes it.
Why does my breath smell on Mounjaro?
The three usual causes are ketone breath from eating very little, a dry mouth letting bacteria build up, and reflux or sulphur burps from slower stomach emptying. The NHS lists fasting and crash dieting as a cause of bad breath. Eating regular small meals with some carbohydrate often clears ketone breath within a few days.
What does ketone breath smell like?
People describe it as fruity, sweet or like nail polish remover. It happens when the body burns fat for fuel because you are eating very little, especially very little carbohydrate. In someone with diabetes who also feels very thirsty, sick and drowsy, this smell can be a sign of diabetic ketoacidosis and needs urgent care.
Can dry mouth on Mounjaro damage my teeth?
Yes. Saliva protects teeth against acid, so a dry mouth raises the risk of decay. Brush twice a day with fluoride toothpaste, spit rather than rinse, cut down sugary drinks and see your dentist for regular check-ups.
Does sugar-free gum help?
Yes. The NHS recommends chewing sugar-free gum or sucking sugar-free sweets for dry mouth, and sugar-free mints or gum for bad breath. Chewing gum also makes you swallow more air, so go easy if you already have bloating or burping.
