In short

  • Each KwikPen holds 4 fixed weekly doses. Throw it away after the fourth dose
  • Throw a started pen away 30 days after first use, even if doses are left in it
  • A pen that has been frozen must be thrown away, even if it looks normal
  • Used needles go into a sharps bin straight after the injection. Loose needles never go in household waste
  • Do not recycle a sharps bin, and do not fill it above the fill line

Take the needle off your finished Mounjaro KwikPen and put the needle straight into a sharps bin. Then put the pen in the sharps bin too, or take it to a community pharmacy for disposal. Do not put a used pen in your household bin, your recycling or down the toilet. A pen is finished after its fourth weekly dose or 30 days after you first used it, whichever comes first.

When a KwikPen counts as finished

The Mounjaro KwikPen sold in the UK is a multi-dose pen. The instructions for use state each pen holds 4 fixed doses, one taken each week, and tell you to dispose of the pen after those 4 doses. A second limit applies from the day of your first injection.

SituationWhat to do with the pen
You have taken the fourth doseDispose of it
30 days have passed since the first doseDispose of it, even if doses remain
The pen has been frozenDispose of it, even if it has thawed
A started pen has been kept above 30CDo not use it. Dispose of it and ask for a replacement
The pen is cracked, leaking or past its expiry dateDo not use it. Dispose of it

The 30-day rule catches people out when a dose is missed or delayed. If you skip a week, the fourth dose may fall after day 30. The pen has to go at day 30 anyway. Speak to your prescriber if a gap leaves you short, and do not stretch the pen.

Some people notice liquid left in the pen after the fourth dose and try to draw out a fifth. The pen is not designed for this and the amount is not a measured dose. The guide to the fifth dose and avoiding a gap explains why, and what to do instead.

Why household waste is the wrong place

A used KwikPen is medicine waste. It still holds traces of tirzepatide, and the body houses a glass cartridge. If the needle is still attached, the pen is a sharp as well.

Bin crews and recycling staff handle waste by hand, and a hidden needle can injure them. Medicines thrown in household bins can also reach landfill and water supplies. NHS Grampian's guidance on unused medicines says they should never be flushed down the toilet or put in a household bin, and should go back to a community pharmacy.

The plastic pen body looks like ordinary packaging, so recycling is a common mistake. Keep pens out of your recycling bin. The outer cardboard box and the paper leaflet are fine to recycle once the pen is out.

Step by step: disposing of a finished pen

  1. After your injection, hold the pen with the needle pointing away from you and follow the needle removal steps in the instructions for use.
  2. Unscrew the needle and drop it straight into a sharps bin. The instructions for use say to do this right away after each injection.
  3. Put the pen cap back on the pen.
  4. If the pen fits through the sharps bin opening, place it in the bin. If not, keep it somewhere safe and dry and take it to a community pharmacy.
  5. Close the sharps bin lid between uses. Seal it fully once it reaches the fill line.

The KwikPen how-to page covers the injection steps before this point, including attaching a new needle each week.

Sharps bin or pharmacy: which one

Both are safe. The choice depends on what you have at home and what your local services accept.

Sharps bin at home

A sharps bin is a rigid yellow container with a lid and a locking closure. Many people put both needles and finished pens into the same bin. Needles for the KwikPen are not in the box, so you buy them separately, and some suppliers send a small sharps bin with the first order. The needle disposal guide explains where to get a bin, how full to fill it and how to get a full one collected.

Kent Community Health NHS Foundation Trust advises filling a bin no more than three quarters full and never past the fill line. Do not press items down to make room. Once the bin is sealed, keep it somewhere dry, out of reach of children and pets, until collection or drop-off.

Pharmacy take-back

Community pharmacies accept unwanted medicines from the public for safe disposal. A finished pen with the needle removed falls in this group. Take it in the cap, in a bag or small box, and tell the staff it is a used injection pen.

Full sharps bins are a different matter. Some pharmacies accept sealed bins, others do not, and arrangements differ between areas of the UK. Ask before you bring one in. If your pharmacy says no, your local council is the next place to try.

Council collection

Many councils run a clinical waste or sharps collection from your home. You book it online or by phone. Rules vary: some councils ask for a GP referral before the first collection, some charge, and some ask you to leave the sealed bin in a set place on collection day. Search your council website for "clinical waste" or "sharps collection" to find the local rules.

What to do with pens you never used

You may end up with an unopened pen you no longer need, for example after stopping treatment or moving to a different dose. Keep it in the fridge until you decide what to do. Do not sell or give it to someone else. Mounjaro is a prescription-only medicine, and the dose and safety checks were made for you.

If you will not use it, return it to a community pharmacy in its box. If you are moving between doses, ask your prescriber first. Some people move up using a pen they already hold, which only works if the prescriber agrees.

An unstarted pen left out of the fridge should not be used. The leaflet gives no out-of-fridge allowance before first use, so keep it refrigerated until you start it. If you are unsure whether a pen is still safe, ask your pharmacist.

Disposing of pens away from home

If you travel for 2 weeks or more, a pen may finish while you are away. Pack a small travel sharps bin in your bag, and keep the needle and the finished pen in it until you are home. Many countries have their own rules for sharps, so do not leave a used pen in a hotel bin. The guide to travelling abroad with Mounjaro covers packing pens and needles for flights.

Signs a pen should not be used

Check the pen before each injection, not only at the end. Do not use it if:

  • it has been frozen at any point
  • the expiry date on the label has passed
  • the pen is cracked, damaged or leaking
  • the liquid looks cloudy or contains particles
  • a started pen has passed 30 days or was kept above 30C

Pens bought from unregulated sellers are a separate risk. The MHRA has warned about fake weight loss pens sold online. The page on fake Mounjaro pens lists what to look for. If you suspect a fake, do not inject from it. Keep the pen and its packaging and report it through the MHRA Yellow Card scheme, then dispose of it once you have reported it.

If someone is injured by a used pen or needle

If a child or another person pricks themselves on a used needle, encourage the wound to bleed, wash it under running water with soap, and cover it with a waterproof plaster. Do not scrub or suck the wound. Then call 111 or go to A&E for advice the same day. The needle disposal guide explains why this matters more when the needle belonged to someone else.

A pen with the needle removed carries no sharps risk. If a child swallows liquid from a pen or injects from one, call 111 or your GP straight away, and take the pen with you if you need to be seen.

Questions people ask

Can I put an empty Mounjaro KwikPen in the bin?

No. A finished pen still holds traces of medicine and, if the needle was left on, a sharp. Take the needle off, put the needle in a sharps bin, then put the pen in the sharps bin or take it to a community pharmacy. Household waste and recycling are not suitable.

Can I recycle the plastic from a KwikPen?

No. The pen counts as medicine waste once used, and the instructions for use say not to recycle a sharps container. Recycling staff sort waste by hand, so a pen or needle in the wrong bin puts them at risk.

Will a pharmacy take my used Mounjaro pens?

Community pharmacies accept unwanted medicines for safe disposal, and the NHS asks you to return medicines there instead of binning or flushing them. Policies on full sharps bins vary, so ask your local pharmacy before you bring one in.

What do I do with a pen that still has doses left after 30 days?

Throw it away. The 30-day limit starts from the first injection and applies even if one or more doses remain. Dispose of it the same way as an empty pen, and speak to your prescriber if you keep running past 30 days.

What if my pen was frozen or left somewhere hot?

A frozen pen must be thrown away, even after it thaws. A started pen kept above 30C, or an unstarted pen left out of the fridge, should not be used either. Dispose of it safely and contact your prescriber or the supplier about a replacement.
