In short

  • Pharmacies set their own private prices for Mounjaro. There is no fixed private price
  • Each KwikPen holds 4 weekly doses, so the cost per dose is the pen price divided by 4
  • Eli Lilly raised its UK list price for private supply from 1 September 2025. NHS supply was not affected
  • Higher strengths usually cost more per pen than lower strengths
  • On the NHS in England you pay the standard prescription charge unless exempt. Prescriptions are free in Wales, Scotland and Northern Ireland

When you buy Mounjaro privately, the price covers four things: the pen, the prescriber's assessment and ongoing reviews, dispensing by a registered pharmacy, and chilled delivery to your door. Pharmacies set their own private prices, so the same strength costs different amounts from one service to the next. Higher strengths usually cost more, and the first order is often priced differently from repeats. For live figures, compare UK Mounjaro prices by dose.

What sits inside a private price

The medicine

Mounjaro is made by Eli Lilly and reaches pharmacies through licensed wholesalers. The manufacturer sets a list price for private supply, and pharmacies buy at or around that level depending on their terms. Lilly raised its UK list price for private prescriptions from 1 September 2025. The Department of Health and Social Care said NHS supply was not affected, because the NHS buys under a separate arrangement.

Each KwikPen holds 4 doses, one a week. The patient leaflet says any medicine left in the pen after the fourth dose should not be used. So one pen equals 4 weeks of treatment.

The prescriber's work

A prescriber has to assess you before Mounjaro can be supplied. Since February 2025, GPhC guidance has required online services to:

  • check your identity
  • verify your weight, height or BMI by video, in person or from your records
  • have two-way contact with you, not rely on a questionnaire alone
  • ask consent to share the prescription with your GP
  • review your progress at intervals on repeat orders

Video checks and prescriber time cost money. Some services include them in the pen price. Others charge a separate consultation fee, either once or with each order.

Dispensing

A pharmacist checks every prescription before supply. The pharmacy labels the pen with your name and dose and keeps records. These costs sit inside the price.

Delivery and packaging

Mounjaro must stay at 2C to 8C until you start it. Pharmacies use insulated boxes with ice packs and next-day or timed couriers. Some include delivery in the price. Others add a fee, which matters most when you order one pen at a time. The guide to delivery options explains what to check when the parcel arrives.

Extras

Some services include pen needles, a sharps bin or access to a dietitian or support app. Others sell these separately or do not offer them.

Why the same pen costs different amounts

Every legitimate pharmacy supplies the same licensed product, yet prices differ for several reasons:

  • buying terms with wholesalers and the manufacturer vary between pharmacies
  • some pharmacies accept a lower margin on the pen to attract new customers
  • consultation and delivery fees are included by some services and added by others
  • larger services spread fixed costs, such as video checks, over more orders
  • prices change at different times, so a gap opens after any list price change

A lower price does not mean a weaker service, provided the pharmacy is registered and makes the checks above. The guide to buying Mounjaro safely lists what a legitimate service must do.

Why higher doses cost more

The UK private price usually rises with strength. A 15mg pen contains six times the tirzepatide of a 2.5mg pen, and the list price for higher strengths is higher. Each pen still holds 4 doses.

The cost of treatment therefore changes as your dose rises. Under the SmPC schedule, you start on 2.5mg for 4 weeks, then move to 5mg, and can rise in 2.5mg steps every 4 weeks or more. When you budget, look at the price at the dose you expect to settle on, not the starting pen. Your prescriber decides when, and whether, to move you up.

First order and repeat prices

Many services set a lower price for a first order. This helps you start, but you pay the repeat price for every pen after. Over a year of treatment the repeat price shapes your total cost far more than the first order does.

When you compare, note the repeat price for each strength you are likely to need, and whether the service changes its prices without notice.

Bundles and subscriptions

Bundles

A bundle is 2 or 3 pens in one order, usually at a lower price per pen and with one delivery charge. Bundles suit people settled on one dose. During dose increases they can leave you with pens at the wrong strength, and dispensed medicines generally cannot be returned. The guide to Mounjaro bundles covers when they make sense.

In August 2025, ahead of the list price rise, the Royal Pharmaceutical Society strongly advised against stockpiling, citing pressure on supply. Never order from several providers at once. Each prescriber needs a full picture of what you take.

Subscriptions

Some services send a pen every 4 weeks on a rolling plan. Before you sign up, check:

  1. How to pause or cancel, and how much notice you must give.
  2. What happens when your prescriber changes your dose.
  3. Whether the price is fixed or can change during the plan.
  4. Whether each order still includes a review of side effects and weight.

How to compare like for like

Use the same basis for every service you look at.

CheckWhy it matters
Registered with the GPhC (or PSNI)The pharmacy is legal and inspected
Repeat price at your doseYou pay this for months
Delivery fee per orderAdds to every single-pen order
Consultation or review feeSome charge once, some every order
Cost per weekly doseTotal price, including fees, divided by the number of doses
Cancellation and refund termsMatter if your dose changes or you stop

Cost per weekly dose is the fairest single figure. Add the pen price, delivery and any fees, then divide by 4 for a single pen, or by 8 or 12 for a bundle.

Switching to save money

Moving to a cheaper pharmacy is allowed, but the new prescriber has to assess you, verify your weight and confirm your current dose. A gap in supply can mean restarting at a lower strength. The guide to switching pharmacy on Mounjaro explains the records to bring and how to time the move.

Prices that look too low

Registered pharmacies pay for the medicine, the checks and cold-chain delivery, so their prices sit within a range set by those costs. The MHRA has seized counterfeit pre-filled weight loss pens and warns against buying from social media or beauty salons. A pen offered without a prescription, or at a price no registered pharmacy can match, is a warning sign.

The NHS cost

NHS supply follows a different system. If you qualify under the NHS rollout, you do not pay for the medicine itself. In England you pay the standard prescription charge for each item unless you are exempt, for example because your diabetes is treated with medicine. Prescriptions are free in Wales, Scotland and Northern Ireland.

Questions people ask

Why does Mounjaro cost different amounts at different pharmacies?

Each pharmacy sets its own private price. The price reflects what the pharmacy pays for the medicine, its prescribing and checking costs, its delivery method and how it chooses to compete. Some charge separately for delivery or consultations, so the headline pen price is not always the full cost.

Why do higher doses of Mounjaro cost more?

The manufacturer's list price for private supply is higher for the larger strengths, and pharmacies pass this on. Each pen holds the same 4 doses, but a 15mg pen contains six times as much tirzepatide as a 2.5mg pen.

Is the first Mounjaro order cheaper than repeats?

Often. Many services price the first pen lower to win new customers. Since you will pay the repeat price for months, compare repeat prices at each dose you expect to reach, not the first order alone.

Are Mounjaro subscriptions worth it?

They can save time, but read the terms. Check how to pause or cancel, what happens when your dose changes, whether the price is fixed and whether each order still includes a prescriber review.

Is a very cheap Mounjaro pen a warning sign?

It can be. Legitimate pharmacies buy from licensed wholesalers and pay for prescriber checks and cold-chain delivery. A pen sold without a prescription, through social media or at a price no registered pharmacy offers may be fake, and the MHRA has seized counterfeit weight loss pens.

Does Mounjaro cost the same on the NHS?

No. NHS supply is not affected by private list prices. In England you pay the standard NHS prescription charge for each item unless you are exempt, and prescriptions are free in Wales, Scotland and Northern Ireland. Only people who meet the NHS criteria can get it this way.
